After selecting "Submit," a screen will appear to inform you if your submission was successful. You can then use the "Back" button on your broswer to return to this screen.

(14a) If paying by CREDIT CARD (Visa or Mastercard):

3. State whether you are using Visa or Mastercard
4. Give the 16 digit account number
5. Give the expiration date
6. Spell the name EXACTLY as it appears on the card
